Abstract :

Heparin and Low molecular weight heparin (LMWH) had been used widely for the prevention and treatment of thrombosis in patients with acute coronary syndrome (ACS). Actually, the administration of heparin may cause some adverse effect, such as heparin induced-thrombocytopenia (HIT). This study was aimed to know the reveal thrombocytes count in patients with ACS who underwent LMWH therapy.  An observational study with cohort prospective design was performed in 30 patients with ACS. The thrombocytes count was obtained from complete blood count (CBC) by using haematology analyzer (Sysmex XT 2000i) that performed at the Clinical Pathology Laboratory, Dr Wahidin Sudirohusodo Hospital, Makassar. The obtained data was analyzed with Wilcoxon test. The mean thrombocytes count before the administration of LMWH was 236.800/µL, while the mean thrombocytes count after the administration of LMWH was 280.270/µL. Surprisingly this data showed that in general the  thrombocytes count was increased significantly ( P = 0.004 ) five day after starting LMWH therapy. In this study only one (3.3%) of the 30 patients who received LMWH had trombocytes count less than 150.000/μL. Based on this study an antibody test to PF4-heparin complex was needed to determine the occurence of HIT in this patient. The thrombocytes count was increased significantly (P = 0.004) in patients with ACS who had given LMWH therapy.       Heparin dan heparin berberat molekul rendah/low molecular weight heparin (LMWH) telah dipakai secara luas untuk pencegahan dan pengobatan trombosis di penderita Sindroma Koroner Akut (SKA). Pemberian heparin ternyata mempunyai beberapa pengaruh samping, salah satunya adalah trombositopeni imbasan heparin/heparin induced-trombocytopenia (HIT). Penelitian ini bertujuan untuk mengetahui jumlah trombosit penderita SKA yang mendapat pengobatan LMWH. Penelitian dilakukan secara amatan dengan rancangan penelitian selidikan kelompok terpilih (kohort prospektif), di 30 pasien SKA. Data trombosit diperoleh dari periksaan uji darah lengkap (CBC) menggunakan penganalisis darah/hematology analizer (Sysmex XT 2000i) di Laboratorium Patologi Klinik RS. dr. Wahidin Sudirohusodo Makassar. Data kemudian dianalisis dengan uji Wilcoxon. Rerata trombosit sebelum pemberian LMWH adalah 236.800/uL, sedangkan rerata trombosit sesudah pemberian LMWH adalah 280.270/uL. Data ini menunjukkan bahwa secara umum terjadi peningkatan jumlah trombosit yang bermakna secara statistik (P=0,004) setelah lima hari pemberian LMWH. Hanya satu dari 30 subjek penelitian yang memiliki hitung akhir trombosit kurang dari 150.000/μL. Untuk membuktikan bahwa HIT di penderita ini diperlukan pemeriksaan antibodi terhadap kompleks PF4-heparin. Peningkatan jumlah trombosit yang bermakna secara statistik  terjadi di pasien SKA yang diberi LMWH.
